Registration Requirements.

All patients are required to use a unique Personal Identification Number (PIN) to verify their identify. You should have received a PIN at your last office visit.  If you have not received your PIN, please contact your doctor's office to request one.  

Registration Process

Once you have have received your PIN, please note that you will need to complete all 3 registration steps in order to start receiving secure messages from our office:

1: Establish a login by creating your own user ID and password.

2: Verify your identity using the PIN.

3: Submit your preferred method of contact
